Accessing Your Medical Records
Patients may request copies of their medical records for personal use, continuity of care, or to share with another healthcare provider. Requests can be made by completing our Medical Records Request Form and submitting it to your CareGap MedFast Clinic location.
For your privacy and security, all requests are processed in accordance with applicable healthcare privacy laws.
